CHC roundtable: Reimagining the hospital to home journey

As part of our Senedd election campaign, we are convening a virtual roundtable session on reimagining the hospital to home journey.

The goal is simple: Keep people healthy, independent, and in their own homes for as long as possible. The reality is more complex: a system where "getting home quickly" is often the biggest hurdle to recovery. In the roundtable we will explore:

• Thinking creatively about how we can maximise the use of new and existing homes to reduce delayed discharges via step down and other support offerings.

• Preventing readmission via home support and reablement services.

• Building trust and a culture that prioritises getting people home across a partnership
